FIRST TIME VISITS Checklist
INTAKE FORMS
Your therapist will provide you with our intake forms electronically through our online Portal. For your convenience, these documents are in a type-able PDF format that allows for electronic signatures. There’s no need to have a printer or scanner or to fill out anything by hand. And the documents are always maintained within medically encrypted and secure system. Most people find it takes about 10 minutes to read through the materials.
These documents also include a brief questionnaire that gives you the opportunity to reflect upon what might be important for me to understand about you as we begin our work together. This questionnaire may take 30 minutes or more to complete thoroughly. You can provide as little or as much detail as you like. The more information you’re able to share with your therapist, the more quickly they will be oriented in how to best help you.
The intake documents also include some material similar to what may be found on the website about clinic policies, as well as what to expect in psychotherapy, including our training and approach.
Your therapist will be happy to discuss any questions that you might have regarding the forms or the counseling process at our first meeting.
OFFICE VISIT INFORMATION
At Core Psychotherapy, we believe that making the most of therapy involves intentionally stepping out of the pace and demands of everyday life and into a different mode entirely. The more distinct you are able to treat therapy space from your everyday experience, the greater potential there is to make the changes in your life you desire.
Our waiting area includes a unique gallery space across the hallway that serves as a kind of decompression chamber between the outside world and the world of your therapy. The space is designed to foster openness, intentionality, creativity, and contemplation, offering a departure from the Everyday.
We understand therapy as a shared meditative space — a place to observe, with curiosity and without judgment, the Self as it is revealed in relationship.
Many clients find that spending a few moments in the gallery beforehand helps them transition from the busy sights and sounds of daily life into whatever mindset is most conducive to their therapeutic work.
We also invite clients to spend time in the gallery after their session. The space can help support reflection, integration, and a gradual transition back into everyday life.
Whether you choose to engage with the exhibits, write, create, or simply sit quietly, the gallery is available as a place for contemplation, curiosity, and self-expression in whatever form feels meaningful to you.
